Admission Information
Heritage Place was developed and is funded as a supportive housing model.
In order to be considered for admission you must require a degree of supervision and/or assistance with activities of daily living. Based upon the completion of the Inter RAI CHA Assessment (Inter-Resident Assessment Instrument Community Health Assessment), support services are scheduled on an as-needed basis, to maintain your independence.
- You must be able to manage/direct your own care.
- You must be cognitively aware of surroundings and can assist with decision making.
- You must be able to ambulate on your own or with assistive devices and able to transfer yourself.
- You must be independent with your bowel and bladder care.
- You must be independent with eating and the ability to feed yourself.
- You must be able to participate/assist with the activities of daily living.
- You must be able to have your needs met by visiting health care professionals.
Members of the Beaver Rebekah or Odd Fellow Order are given priority over non-members on the waiting list.
