What is the application process to obtain a spot in a long-term care Home?

The steps in the application process are:

 
  • If you wish to apply for admission to and be placed on a waiting list for a Long-Term Care Home in Simcoe County, please contact the local LHIN Home and Community Care division:

  • The CCAC staff will guide you through the application process and choice of Homes. They will encourage you to tour the Homes you are interested in order for you to make an informed choice.

  • If you may wish to take a tour of our Home to assist your decision-making, please call the IOOF Home Reception at 705 728 2389 Ext. “0” to book an appointment for a tour.

  • Once approved for admission, the LHIN Home and Community Care will stay in touch regarding expected date of admission.Upon admission to the Home, the care team will begin an assessment of care needs, in conjunction with the new Resident and his/her family/or significant decision maker (SDM)Once admitted, Home staff will provide the new Resident and his/her family or SDM with an orientation to the Home and assist with transition into the Home.

What is the cost of long-term care?

Rates for living in a long-term care home are set by set by the Ministry of Health and Long-Term Care. You can obtain this information from their website or by contacting the local Home and Community Care at the North Simcoe Muskoka LHIN.
